Pressure injuries are a frequent and costly problem among hospitalized adults and are associated with immobility, moisture, malnutrition, medical devices, and the insufficient or delayed implementation of preventive nursing care. The objective was to characterize the available scientific evidence on nursing interventions used to prevent pressure injuries in hospitalized adult patients. A qualitative-descriptive scoping review was conducted, guided by PRISMA-ScR and the PCC framework. Thirteen original studies were selected, and the evidence was organized through descriptive analysis, a charting matrix, and thematic synthesis. The studies showed a predominance of multicomponent interventions in intensive care settings, integrating risk assessment, skin surveillance, repositioning, moisture management, medical device management, and education, with generally favorable but methodologically heterogeneous findings. Effective prevention requires integrated, individualized, and institutionally sustained nursing care, while future research should strengthen comparability, multicenter evaluation, sustainability, and applicability across different adult hospital services.
