Adherence to nonpharmacological treatment in people with type 2 diabetes mellitus is a complex process influenced by emotional, sociocultural, and contextual factors, in which subjective experiences shape self-care practices. The objective of the study was to explore perceptions of adherence to non-pharmacological treatment among people with type 2 diabetes mellitus. A qualitative study with a phenomenological approach was conducted with five older adult women, using semi-structured interviews designed to explore meanings, emotions, and experiences related to daily self-care. Thematic analysis allowed for the identification of categories that reflect how participants make sense of the disease, experience emotional responses, and shape their adherence practices. The results suggest that the experience of the disease is associated with emotional processes that can hinder the sustained adoption of behaviors such as healthy eating and physical activity. Furthermore, non-pharmacological adherence is mediated by factors such as economic status, family support, professional guidance, and the sociocultural context. It is concluded that adherence to nonpharmacological treatment is a dynamic and multidimensional process, and understanding it through people’s experiences provides valuable insights for strengthening nursing care in chronic diseases.
